According to a 1997 Canada Health Monitor Survey, 56 percent of Canadians take some sort of natural health product. And while only 20 percent reported taking herbal remedies, that proportion had doubled from a survey only three years earlier. Judging by the burgeoning shelves of natural remedies in pharmacies, and even grocery and department stores, more Canadians than ever seem to be giving alternative remedies a try.

So how do you weed out which herbal preparations might be right for your health needs?

Myth: Herbal remedies are 'natural'; conventional drugs are not.

A large number of drugs on the pharmacy shelf do come from Mother Nature, notes naturopathic doctor Paul Saunders. For example, "digoxin is still extracted from plants -- that's the most efficient way to make it," he explains. Pharmaceutical giants like Merck have also marketed herbal remedies like Echinacea.

The big difference? In many cases, we still don't have enough long-term studies on the safety and efficacy of herbal remedies to make solid conclusions.

Myth: Herbal remedies have no side effects.

Just like other medications, most herbs have possible side effects and they can also interact with conventional medications, so inform your doctor about any herbal remedies and supplements you're taking. (Be sure to speak up of you're booked for surgery -- some herbs may trigger bleeding or interact with anesthesia.)

Herbs can react with medications in two different ways, explains Dr. Edmund Lui, an associate professor of physiology and pharmacy at the University of Western Ontario. Some herbs may alter concentrations of other medications in the blood, so that it either builds to potentially dangerous levels or simply doesn't work.

Other herb/drug combinations interact to changing blood levels of the medication.

Myth: All drug/herb interactions are potentially dangerous.

Just as certain drugs are sometimes used together, certain herb/drug combination may offer benefits beyond those either could offer alone. For example, Dr. Lui is exploring the possibility that certain herbs could dampen the toxic effects of chemotherapy drugs.

My doctor might get upset if I take herbs

Dr. Lui says he has watched the medical students coming through the University of Western become much more open to the idea of alternatives and complementary therapies, and many medical schools now at least touch on these topics. Tell your doctor about any supplements or herbal remedies you're taking so she/he can avoid problems.

Shopping for Herbal Remedies

Health Canada has formed the Office of Natural Health Products, which will ensure preparations like herbal and homeopathic remedies sold in Canada meet strict criteria.

In the past the amount of the active ingredient in some products did not match the label information. Occasionally capsules or tinctures were tainted with potentially harmful substances or undeclared pharmaceuticals. The process of setting up a system of checks and balances is still underway.

In the meantime, here are some shopping tips:

• Buy recognized name brands or store brands.
• Opt for preparations labeled "standardized" where possible.
• When in doubt, ask your pharmacist, naturopathic doctor or physician.

According to a 1999 report from the American Botanical Council, gingko biloba leaf is the top selling herb in the U.S. It is widely used to improve memory function and to treat Alzheimer's disease and other dementias. Available in a standardized oral extract, gingko is usually taken three times daily, starting at a total of 120 mg.

"The literature isn't there to say definitely," says Mary Beth Blokker, but the plant preparation may modestly improve functioning in patients with dementia, and otherwise healthy people who have mild to moderate age-related memory loss.
